It gets worse.  I just visited my German office in Munich and could never get
dialout service of any kind.  I have a Mac Powerbook with an internal modem. 
Guess what?  My modern office is ISDN.  Guess what?  My modem does not speak
ISDN.  In fact, not too many modems speak ISDN.  So, ISDN has a huge hurdle to
overcome for individual usage.  If you are inside a corporate network you can
afford the gateway/bridge cost of translating to/from ISDN and the old modem
world on a PPP/SLIP link.  

I can't believe that going forward goes backwards so much!
