All of the above. I agree that ISDN would be good. But even more than that,
I think the $900-1000 boxes needed on each end are a major stumbling block.
I see in EE Times that the cable companies just got a rude surprise from
the set-top decoder folks: Those $399 (retail) set-top 500 channel decoders
are looking more like $700 just to manufacture (not counting software
licensing fees!)
So I'd be more excited by a $299 CSU/DSU than by a few more % points of
penetration...
