Tariff and POP Questions

Here are a few questions. I'd appreciate any answers or directions to
sources of answers.
1. Which carriers have issued T-3 TCP/IP tariffs? Has MCI done so yet for
the new NSFnet backbone.
2. Must such tariffs by their very nature be application and geography
specific? IE AT&T would not say it is x dollars per mile no matter where
in the country you are?
3. Is it correct to assume that no one has issued an OC12 tariff? When
and under what conditions might this be likely to happen for the first
time.
4. How many POPs does MCI have in its nationwide network?
