Problems Printing Richard Mandelbaum's Strategic Futures Document

I just received an electronic Postscript copy of Richard Mandelbaum's
"The Strategic Future of the Mid-Level Networks." I very much want to
read what appears to be an excellent document, but upon printing I
discover that pages 30-39 are skipped, and printing begins again on
p. 40. Did others experience this same problem, and where can I FTP
another copy of the document to verify that the problem is in the
document as sent and not with the particular version that I received?
If the problem is with the document, then perhaps someone could send
me a version of the document that prints all pages? Please confirm
by mail that I have not already received a copy of the document before
sending in order to save network bandwidth.
