I'm interested in anybody offering "unrestricted traffic" access
on essentially a common carrier basis. Interested in regionals,
international, too.

Jim,
You're asking for something that's quite reasonable, but very hard to
provide today. There are a number of network service providers which
provide basically "unrestricted traffic" within their networks, but there
is no general assurance that you can exchange unrestricted traffic between
any two.
There are at least three initiatives underway to interconnect the various
commercial islands but until that occurs you have to check with each network
to determine who they can exchange unrestricted traffic with.
