RVD server returns bogus error code

Spinup returned a bogus error message when I tried to spinup vssys on 
praxiteles.  This has been tested on both a 7.2P machine and a 6.0C
machine --- not that it matters, since I'm fairly sure it's a problem
with the RVD server, not the client program:

12% spinup 1 praxiteles vssys r
spinup: Unknown spinup mode.
While attempting spinup of
    pack vssys at server praxiteles on drive 1 in read-only mode.

Apparently operations had changed the modes on the rvd to 0 in
preparation for an update (or something like that), and the RVD server
was passing back a bogus error message when it couldn't open the rvd
file/device.

As a related issue, attach -d should be modified to print out the error
messages from spinup; it would have made debugging the problem with
w20a-vssys much easier.
