When compile_et processes an error table like:

	error_table test
	ec XXX=0, "error 0"
	ec YYY=9, "error 9"
	end

where there are gaps, it initializes the missing entries (1-8 in this case)
to "(char *)NULL", but does not include <stdio.h> or define NULL on the .c
file.
