"sh: 4913 Bus error" message on login

This came from an olc user, so this is all the info I've got.
She logged on to a workstation and her main window just disappeared, and she
did not get the logout button. However, the workstation was successfully
receiving z-grams sent to her. (She logged in to an adjacent workstation
to get to olc.) Also,
right before the "session init. cmpleted" line it says
"sh: 4913 Bus error"
for what that's worth.
The olc log continues...

Hmm, that's a funny error message. I take it that most of the time, you can
log in normally, and this problem only happened once?

yeah, most of the time it's fine.
this machine had one of those can't-find-a-tty error msgs on it before i
logged on, but i've always succesfully ignored them. i don't know what
was wrong.
i restarted it, and now it says "hit any key to start", like it's supposed
to.
That's it. The username is 'debbo' should you want to ask her any more
questions. The workstation in the log is NOT the workstation that showed
the problem; the problem happened on some workstation nearby.
