Problem:
An application installs its own colormap via XSetWindowColormap.
If it dies from an XError, the window manager does not uninstall
the colormap when the window is unmapped.
What should have happened:
The application's colormap should have been uninstalled and replaced
by the colormap associated with the window that the pointer is now
in (or the Default if on the root)
