9.4.27 linux: firefox does not handle .ram, etc., appropriately

User grfilcht came by SIPB with problems dealing with RealAudio in
firefox. While she had some special-case difficulties resulting from
prior configuration, starting her off with a clean configuration
(removing .mailcap, .mime_types, and .mozilla) did not solve the
problem.
Similarly, testing as sipbtest, with none of the above configuration
files/directories, downloading a .RAM file (for instance,
http://real.xobix.ch/live/ssatclass.ram, with mime-type
audio/x-pn-realaudio) causes firefox to prompt the user to save or
browse for an application.
But instead (and previously the default was, I thought!) it should run
realplay from the infoagents locker.
I see the same behavior with multiple accounts when running
env HOME=/var/tmp firefox &
which should be sufficient to give the default configuration.
So, in summary, firefox's default mime types don't seem to be right.
about:config suggests it is using mime.types and mailcap from /etc,
though I am often confused about whether the .rdf file is used
instead, etc.
