Please consider the following changes to improve this man page:
Note that init launches toehold from /etc/ttys. This is rather obscure
(in the sense that it is the only process besides getty that is
started up this way). Thus I suggest mentioning the *file* /etc/ttys,
as in a FILES section listing /etc/ttys. The SYNOPSIS should mention
the argument that is given to toehold (see /etc/ttys). The document
"Toehold: a Technical Description" you will not find on-line. Thus
either the doc should be put on-line or the man page should not
mention it. Thank you for addressing this matter.
