If you look at the user's mail message it was from "intruder". That
means he was no longer in /etc/passwd. "inc" is only one of many things
that will begin to fail when this happens.
The cause of him not being in /etc/passwd is probably due to him logging
in twice and logging out of the first session (which created him, and
which knows to delete him when he logs out).
Since you have the question, I'll let you follow-up on it.
