ALL: mset/tn3270 doesn't seem to interpret /etc/map3270 properly

in particular, the entry for xterm in /etc/map3270 provides for
support for the cursor keys and for ^vi keys, but (my opinion here) it
seems that mset isn't doing the right thing with the ^]]A-type
character combinations (it doesn't work when they're the only support
shown, either)
