What should have happened:

Second, the mailer should have returned the message as
undeliverable.
Why? The message was delivered successfully to all specified
recipients.
(In other words, it is not entirely obvious that it is an error for a
message to have no specified recipients.)
I don't have my RFC handy, but I'm pretty sure it's a violation
of 822 to have a message with no recipients. That being the
case, the message should have been returned to the sender.
