I have tracked down the bug in psrev (you can remove your file). The
problem stems from two things:
a) Your file declares that it is PS-Adobe compliant, but then is not
compliant in that it does not insert %%Page: directives (which the
format expects)
b) Even if I fix the bug in psrev, it will not work on your file because
of (a) above.
I will wrap up a bug fix to prevent core dumping something (I have a
copy of your file), but don't expect much.
