Mike,
I've seen similar behavior with versions of 12.x(?) on a 7140.
Extended access-lists failed to work.
Simple access-lists continued to work.
After upgrading to 12.1(2)E1, the problems went away.
Maybe this same issue made it back into 12.1(4) or
maybe these issues are specific to some interaction
with certain hardware...
