Comment:

 This particular screw case seems to be a PDF created with pdftex that was
 then munged by hand with PDFKit on Lion, and may well fall into the "Don't
 do that category."  We're not the only people being screwed by PDFKit on
 Lion -- JetBlue and American have been having boarding pass issues for a
 while now (see https://twitter.com/JetBlue/status/211136438488670209,
 among others).  So I'll keep this open, but it's unclear we can do much
 about it.
