Have a look at survival4, the software by Terry Therneau 
for fitting the PH and other survival models.
Although it does not permit gam() like smoothers, one
can use B-splines via bs() and ns() in the formula language.

In addition, there is hare and heft by Charles Kooperberg that
fits similar nonparametric models via regression splines.
