New telnet integrated

I've integrated the latest telnet from Paul and Dave Borman. It now lives
in appl/telnet/ {telnet, telnetd, login}; the old one is in appl/old-telnet/
the v5 code will probably not work properly until Ted is finished with his
changes, but otherwise it should work. Note that the authentication option
negotiation is incompatible with previous versions (but now is closer/at the
draft spec).
