We could remove the ENABLE_THREADS conditional.  But on further
inspection, this doesn't really get rid of much code, and it's pretty
much all in k5-thread.h and util/support/threads.c.  So it may not be
worth the cost (which would be making life slightly harder for obscure
platforms).
