You need the .sty files in:
tex-archive/macros/latex209/contrib/fancyheadings
at one of the CTAN archives:
United States ftp.shsu.edu 192.92.115.10
United Kingdom ftp.tex.ac.uk 131.151.79.32
Germany ftp.dante.de 129.206.100.192
Also, get a copy of makeindex (buried somewhere at the archive)
and build it, then copy to index . I built implement and api
docs using this.
If you get a "foo.idx not found" type message, run index on the
main .tex file.
I'll mail you the .ps files for a small fee...
