Hi,
I have placed a link to the SIPB machine webform in place of the old form
that existed on http://web.mit.edu/IS/help/network/ip.html
I kept an old copy of the original ip.html page in the locker titled
ip-old.html in case anyone has an idea about how to repair the broken form.
I have also reformmatted the page.
