We recently moved that form. The correct URL for the simple activation
form is currently:
http://web.mit.edu/helpdesk/requests/dropreq.html
Or you can start at the MITnet Services page, and choose the appropriate
links. The URL for MITnet Services is:
http://web.mit.edu/is/help/network/
The above URL is probably the best one to bookmark, since it's not going to
change anytime soon.
