IS-HOME MESSAGE: Revised Comment Form

I have revised the comment form (is/lib/comments.html) slightly, mostly to allow easier maintenance of the mail we get:
- added a "Subject" field so the user can say what the
message is about (shown on Subject line in resulting mail;
e.g., "Revised Comment Form" in this message)
- revised what happens when you select a message type:
1) now prompts you before it wipes out any existing message
2) includes terse indicator in resulting email message,
rather than long string (e.g., now "IS-HOME MESSAGE:"
or "IS-HOME CORRECTION:" etc.)
3) moves cursor to Subject field for easy data entry
- consequently reordered fields on form ("From" field first, etc.)
I hope this makes mail management easier (including quicker access to specific messages, because they're now better labeled...)
