pit-manager (e40-342): time to replace hd2 or controller?

You may recall the drive troubles I was having recently. The DEC
field service people came and opened up my workstation, and discovered
that one of the drives had fallen out of its mounting and was coming
into contact with the casing of another drive, or something like that.
They fixed that, but did not replace the drive that was giving errors,
because they did not have the right replacement drive with them;
instead, they reformatted it.
Well, I'm getting errors again:
