w20-575-121, an SGI O2, appears to have a faulty instruction cache. It
repeatedly generates kernel seg-faults while attempting to boot (Bad addr: 0x0,
cause: 0x8<CE=73 PC: 0x803c2ee4 ep: 0xffffc9c8). PROM diagnostics fail in the
ICache 2 test; all other tests are passed.
You may note that early in the boot process IRIX 6.5 on this platform
complains "PCI <> Error (no PCI address logged)"; this is not part of the
problem. It is, rather, a manifestation of IRIX bug number 643802. The
message is innocuous.
