maytag, an O2, is experiencing a disk or SCSI controller problem.
When trying to reinstall it, it complains of timeouts trying to access
various SCSI targets. I've tried power-cycling and reseating the disk
and motherboard. Occasionally it seems to work, but usually not.
maytag is in the E40 basement machine room near the door (turn left as
you go in), and is prominently labeled. It is connected to a serial
switcher to its right. There is no data on the machine which needs to
be preserved, so feel free to try installing it as a test. If you
need to replace the whole motherboard, though, then I think we'll need
to hook up the machine to a monitor and keyboard in order to make it
use a serial console.)
