m16-034-6 has a dead mouse

The mouse on m16-034-6 died while I was using it.  The mouse buttons and
the mouse movement functions were not working.  After checking the mouse
connection plug, I rebooted the machine to logout.  xlogin will now not
start and it falls back to a console login.
