There is a 4 port surface mounted box with jack #2-225-102 on the left side
and 2-225-103 on the right side. Both ports on 103 are working and the top
port on 102 is working. After visiting the closet I found there is no
connection for the bottom port of 102 and a new installation is required if
it needs to be activated.
