I don't think I;ve seen a reply to this, pardon me if someone has
already answered your questions.  

I'm not familar with either of these problems, and they don't really
fall into the area where ops has a lot of experience.  (It isn't too
often that we try to make a bunch of machines sitting in a machine
room play music. :-) If you haven't tried it you might ask about this
in OLC, since they presumably have some experience helping users with
this sort of thing.  I suppose it is possible that there is some sort
of hardware problem with your machine, but I don't know how likely
that is, and hotline should be helping you if that is the case.
