Typhon D disk dead

Starting at about 5 this morning without any warning, the Athena AFS server
Typhon logged numerous disk errors.

We narrowed the problem down to a blown SCSI controller and a bad 2 G disk.
We replaced the SCSI controller and brought the server up with the 3 good
disks.  We tried for several hours to revive the bad disk swapping everything
but the sealed compartment with the platters and heads in it, but were
unsuccessful.

As of now we are starting a tape restore of Typhon D from a backup made on
Thursday 9/28 (data as of late Wednesday night).

The list of volumes on Typhon D is in the info locker in /mit/info/typhon.d.

We will be notifying the users affected using the 'lert' facility.
