themis disk unit 3 (ra90) needs attention

Problem: Disk unit #3 (ra90) is offline

Current state: - the disk has been dropping offline repeatedly over the
past weekend
- there is now no data on the disk
- this morning, the system was unusable, because it was
repeatedly trying and failing to bring unit 3 online.
I booted the system with the disk powered down, so the
system is currently running fine, provided file service
using the other disks.
- after booting the system with unit 3 powered down, I
was unable to get unit 3 to spin up. It is currently
powered down.
I've looked through the logs for July, and I didn't see any indication that
the disk was going to fail. In other words, this is something that happened
suddenly.
