Reason:
We request a W20 KX-5 shutdown, LaVerde's kitchen hood exhaust fan, from 5 PM to 5 AM on Monday evening into Tuesday morning and again on Tuesday evening into and Wednesday morning, June 10th into 11th and June 11th into 12th, 2013 for the replacement of the LaVerde's kitchen hood exhaust fan by One Call Ventilation and MacNichol Electric (Jim Bedard 978-273-7931). The existing fan is being demo'd out of 602Z and the new fan will be installed on the roof. MacNichol will start the new fan sometime after midnight. MIT is asked to witness the lock-outs of the fan circuit breaker by MacNichol/One Call but not to add to the lock-out on either evening. This is as part of the W20 kitchen exhaust systems upgrade project by Elaine Construction (Robin Lorscheider 781-727-8835). Brian Healy 8-8565 is the MIT project manager.
Effect:
Loss of exhaust hood for the duration of this shutdown.
