VS3100 disk problem

Sounds like a disk problem on this workstation.  I told the user I'd
forward this to hotline, and you would probably contact him for more
information.

Question:
Upon returning from a one-week trip, I found this workstation
(madman, a vax 3100) had crashed.  (I had left it running the
standard X login screen).  The message it was displaying on console
began:

   DKBTDRIVER halting at relative address 041A  DKBTDRIVER base addr = 00001754     (etc., showing various registers)

I rebooted, which apparently worked fine, though the following did
end up in /usr/adm/messages :

   May 17 17:28:14 madman vmunix: pid 383: swap: read error from swap device
   May 17 17:28:14 madman vmunix: pid 331: swap: read error from swap device
   May 17 17:28:14 madman vmunix: panic: hard IO err in swap
   May 17 17:28:14 madman vmunix: syncing disks... 1 1 done

After that the messages all appear normal, and it has been working
fine since that reboot.  What's all this about?  The other person
in this office says there's no evidence of a power hit, and no one
had logged in on the workstation itself while I was away.
