Jennifer et alia,

I tried out the form this morning--here are some comments:
The follwoing is the message I received after putting in my e-mail
address--I also tried tat@mit.edu with the same results--will this
happen to everyone? Is there really more than one tat?

Sorry, but we couldn't find an exact match for tat@athena.mit.edu
(reason: multiple
matches) in the MIT student/staff database, so we couldn't generate
name and address
information for you. You'll need to fill in the whole form manually:

Also, can Libraries be added to the list of departments? I hate
checking other!

I like that Hayden Circ is the default location for pick up; should
Archives be added to locations in case some researcher already has an
appointment to use Archival materials?

Could the boxes for last name first name etc. be more condenses
spatially? For example:

First name Last name
______________________	_____________________

Address Phone
________________	____________________

I ask this because one has to use the mouse to go to the next box and
sinice the form is not all on the screen at once and it is awkward.

I like the nice big note field!

One concern Circ staff are likely to have is that they will have no
record of the request so that when users stop by to inquire about
materials they will not have any info. This has been an issue at
Dewey.
