One idea I came up with while I was sitting here...
Write a web page that will poll the students and have them access it.
Link it to a database of their names or ID#'s to ensure each student
only takes it once.  This is one way I can think of it working.

We are getting ready to do a 5 year program review in our Communications

Networking department within Information Technology.   I was wondering

anyone has a questionnaire for students concerning networking in the
residential halls, cable TV, and / or telephone support.  Or any ideas

how to get feedback from the students - ideas for improvement, etc.
