Hi Jeff,
University council wrote one for us some time ago. Originally this was on an actual form that printed out (ticket) when a student would come in. In order for us to take the machine they had to sign off. We've now integrated it into a form in our ITSM solution. Students coming in go to a kiosk and fill out their ticket. Before they can submit they must agree to these terms and type in their name as a signature.
"I acknowledge that repairs on my computer and peripherals are being performed by students. In consideration for these services being performed at no cost to me, I agree to assume the entire risk of any loss or damage to my computer, peripherals or other property entrusted to Tufts Technology Support Center. I hereby release, and hold harmless Tufts University and its trustees, officers, employees, agents and successors and assigns (together, "Tufts") from and against any and all liabilities, claims, damages, causes of action, and expenses, resulting from or related to my use of the services of Tufts Technology Support Center."

Hello,
During the conference this past summer I talked to several of you who have drop off services. When students bring in their computer to drop it off you have them complete and sign a liability form. Would any of you be willing to share such form so I can make sure that I am not missing anything in the one that I am creating?
