We just started using Qualtrics though a site license, and I must say that I like it very much for what little bit I've used it for so far.
It's very powerful, but the interface is pretty easy to use and Erin said, there is good support/training.
That said, for lighter stuff, I have often used Google Forms (part of Google Docs), and it works very well.
