For ResNet and our computer repair center, we use LimeSurvey. I think
our university has a Qualtrics license (subscription? not sure what to
call it), but we haven't used it for our purposes yet. LimeSurvey does
what we need it to do, and the community support is pretty good. I've
never had a question that went unanswered for more than a day or so.
