Hi Jethro,
I've got radiator set up to do either EAP something auth for 802.1 and  
comparing mac address against a mysql data base table entry for mac  
auth. Our HP switches pass the mac address in the user-name radius  
attribute so all i do in the Radiator config file is match the  
username field against a regular expression that checks for a mac  
address with : or - delimiting characters.

As far as the web page is concerned, xforms is your friend. Git a  
simple page with 1 field that the user pasts their mac address into  
which is then inserted into a mysql database
