At Yale, we currently we use a homegrown web application built in Ruby on Rails, called "Reservations." It's open source, so it's free. It's not as robust as I'd like it to be, but major work is being planned this summer.
As far as a general overview, we have the ability to check in and out items, reservations can be made indefinitely into the future and items are broadly categorized into sections like "mics" and "cameras." Currently fees and fines are processed manually, but we are exploring Banner integration over the summer. Students can see what equipment is currently checked out in their name (with plans to give each student access to their checkout history), and are emailed reminders of upcoming check in and outs, as well as overdue items.
I'll be presenting on this application, as well as the general media program merger that was piloted this spring, at the Student Technology Conference at Yale (two separate presentations). Let me know if you want more information, and I'd be happy to email off list or take a phone call or two.
