Followup - House Calls and Liabilitiy Issues

Recently there has been some discussion about ResNet student staff
installing hardware and software into student computers and the liability
issues involved in that process.
My questions is, for those college's that do install network cards and
software for students, do you have the student sign a waiver form of any
sort and what does that look like. And if not, have you had any problems
with this.
