For those of us without LERG access, this information is available (for
free) from
http://www.nanpa.com/number_resource_info/co_code_assignments1.html

You do need to know the CLLI code of the CO to make sense of it, although
in many cases you can figure that out if you know one NPA/NXX for a
location.
