the guy has a week to file a report with the registrar of motor vehicles,
copies to the police of the town where the accident happened and to
his insurance company.
you can get a copy from the police or the registrar. it will contain
the guy's address, and you can ask him the name of his insurance company.
(the police should have noted that down anyway when they asked for his
license and registration -- it's on the latter.)
don't ask how i know.
