I don't have any immediate insights.  If this happens in the
future, pine has a command-line flag that can be used to turn on
debugging output.  "pine -d 4" will enable generic debugging and
"pine -d 9" will enable verbose debugging.  Output goes into a
file named ~/.pine-debug1, and I might be able to look at it to
get clues about what was happening.
