applet to a cgi-script

Java users,

I need to create an applet that would present a form for a user to fill out.
The information in this form would be sent back to the server and processed
by a cgi script.  This cgi script would look up the information in 
a database and present the information to the user in another applet (or possibly 
the same applet?)

Has anyone done something like this?  Is there any code out there that could
show me how to create this kind of user interface and then send the information 
back to the server to do a query?  Then the results of the query be sent back
to the user?

I've seen demos that do something like this, such as the virtual hotel application;
but I have not seen any source code to do something like this.
