NCSA 1.5 comes with that very script. Find in in cgi-bin. I believe it
requires perl.

Dear All,
I have a question about the User Authentication for WWW sever. I am
using the NCSA httpd and user authentication for the access restriction.
How can I setup a process to allow the user to change the password
online? According to the FAQ, there are several CGI script to do it or
setup another WWW sever for the purpose of changing password.
