AFAIK never used in canon. It appears in the TKD Addendum, probably because of this scene at the end of ST5:
[They reach the bridge, where Kirk is confronted by Gen. Koord, now dressed to command the Bird of Prey. Klaa stands off to one side, humbled.]
KOORD: [To the guards] {yIjonHa'!}
KOORD: Kirk, my junior officer has something he would like
to say to you.
KOORD: {yIjatlh!}
KLAA: I... apologize.
[Kirk looks in wonder.]
KOORD: {maj. 'ej...}
KLAA: The attack upon your vessel was not authorized by
my government.
Okrand no doubt came up with {tlhIj} in case the director wanted to film the line in Klingon. As it happened, Klaa spoke his lines in English in the final cut.
