Here's how I understand it after reading the TNG Tech manual
(patvammey vIDelmeH tlhIngan Hol vIlo'laHbe'. tlhoy' Qatlh) :

Dilithium crystals are used to focus the stream of energy that is
created when two streams of particles (one Deuterium, one
anti-Deuterium) are collided in the "warp reactor".

The power is then routed through the plasma conduits to the warp
nacelles, where the power is used to create a warp field (a subspace
bubble) which reduces the mass of the ship and propels it forward.

Your description of the impulse drive is right, fusion reactors are
used with some directional nozzles that basically turns them into
mini-fusion rockets.

The energy from the warp reactor is used to power all the ships
systems. There are also a number of battery backups, in case main
power goes down and some secondary fusion reactors for the same
purpose.

This information is true of most Federation ships, but the science is
the same for Klingons too.
