The following wiki page has the keyboard mapping I used for both the Windows
XP keyboard and the Windows Vista/7 keyboard.

The only thing I did differently in the Vista/7 keyboard was that I put the
pIqaD H on both h and H (in XP it was only on the capital H).
If anyone is interested in downloading the keyboard mapping for Vista/7 you
